How to Apply
Employer:
1) Print and complete the Master Application (PDF)
2) premium payments are by Pre-Authorized bank payment (PDF)
3) attach a copy of your quote, the 1st months premium with a cheque payable to ICBA Benefits
Employee:
1) Print the following enrollment form for each employee (PDF)
2) Print and sign the Personal Authorization and include it with your applications (PDF)
Send original applications, 1st months premium, copy of your quotation to:

What we will do:
1) notify you when we receive your forms
2) review the forms, contact you to obtain missing information
3) once the insurance company has reviewed your medical information we will advsie you of their decision and plan effective date
4) booklets and plan administration kit will be sent by Canada Post in about 30 days from the time the application has been received at the insuance company
5) available to answer questions and offer assistance as required (contact us)
6) in the event your application has been declined you will receive a full refund
7) keep a copy of all documents for your records!
