• the plan is available to individuals, for employees who do
not have an employer plan, and self employed persons and
their employees
• we offer single,couple,and family rates
• coverage ends at age 79
• there are two ways to buy coverage
a) the policy is guaranteed issue, with all benefits taking effect
the 1st of the month without pre-existing conditions if
you apply within 60 days of losing your group coverage
b) or the 1st day of the month following approval of your medical
information
• the Value Plus plan offers $15,000 of guaranteed issue life insurance for
individuals 65 or under (subject to the 60 day enrollment period)
Small Business Value Plan
• the plan is available to individuals, employees who do not have
an employer plan, and self employed persons and their employees

• rates are the same for ages 18 to 64 and from 65 to 79
• coverage ends at age 79
• there are two ways to obtain coverage
a) the policy is guaranteed issue, with all benefits taking effect
the 1st of the month.There are no pre-existing conditions if
you apply within 60 days of losing your group coverage
b) or the 1st day of the month following approval of your medical
information
• the Small Business benefit plan offers $15,000 of guaranteed issue life insurance for
individuals 65 or under
• the ONLY plan in the market place with guaranteed issue long term
disability insurance of $1,000 per month if you are 65 or younger
(subject to the 60 day enrollment period)
• plus an additional $1,500 per month is available for an additional $46 per
month, after the approval of your medical information
