How To Apply-Second Opinion
I have several companies providing competitve Second Opinion. When you and I have discussed the plans and decided on the one suited to your needs I will e-mail you the documents in PDF format.
I will send the following:
Employer Completes and Signs:
a. Master Application
b. Bank Pre Authorization attach a Void Cheque
c. a business Disclosure Statement
2. Each Employee Completes and Signs:
a. Enrollment Forms
b. Personal Authorization giving us permission to speak with their spouse (PDF), or friend
3. What You Will Send
a. fully completed Original applications,
b. signed Bank Pre-Authorization, Void Cheque
c. a cheque for the 1st months premium
d. a signed copy of the Disclosure Statement
e. send your documents to:

What We Do:
a. notify you when we receive your forms
b. review the forms, contact you to obtain missing information
c. booklets and plan administration kit will be sent by Canada Post in about 30 days from the time the application has been received at the insurance company.
