When you have reviewed our recommendations and we
have finalized the plan design and rates. I will send you the
following:
Employer Completes:
1) A Pdf file of the Master Application . The application
must be signed by a company official and witnessed
2) Print and sign two copies of a Disclosure Agreement . This
agreement tells you I get paid a comission from the insurance
company
3) Print,complete and attach a Void Cheque to the bank Pre-Authorized Withdrawal form
Each Employee Completes and Signs:
1) an Enrollment form. A company official signs where applicable
2) a Personal Authorization form (PDF)
3) a Declaration of Insurability
4) send all the original application forms to me along with your 1st months
premium
5) keep a copy of all documents for your records
What we will do:
1) notify you when we receive your forms
2) review the forms, contact you to obtain missing information
3) booklets and plan administration kit will be sent by Canada Post in about 30 days from the time the application has been received at the insurance company
