There are several pre-screening check lists you should review before applying for Long Term Care Insurance
a) If you are currently taking prescriptions or over the counter drugs.
Check this list for some of the medications which could deem you
uninsurable (PDF)
b) Pre-screening Guide –Work sheet (PDF) could also mean you do not qualify
c) height and weight guidelines (PDF)
Once you have read the above guidelines and feel you could qualify for LTC.
Please complete A Quote Request Form and I will contact you to discuss a plan
design
When we have the plan design and completed an application, the Insurance company will request
the following Underwriting requirements:
1) up to age 70- they will complete a phone history interview and do a cognitive screen
2) age 71 to 80- a face-to-face interview and cognitive screen plus obtain and
review your medical records
This process should take 30 days from the time the application is received by the
company and you receive your contract. In order for the insurance company to provide
a contract to you within 30 days, your physician will have to reply promptly to any
requested information.
